What it is

Crowns & Onlays

When a tooth has lost too much structure for a filling, the question is how much more to remove. A traditional full crown takes the tooth down to a peg. An onlay covers only the part that failed.

Aurelia designs toward the second answer wherever the remaining structure allows it. Everything is scanned, designed on screen and milled in the clinic from a single block of lithium disilicate or zirconia, then characterised by hand and glazed.

In most cases you arrive with a broken tooth and leave with a finished one, in a single visit, with no temporary and no second injection.

    Radiographs and transillumination to map the crack or decay and decide honestly between a filling, an onlay and a full crown.

  2. 02

    Preparation

    60 min

    Old restoration and decay removed under magnification. The preparation follows the damage rather than a textbook shape.

  3. 03

    Digital scan

    10 min

    The prepared tooth, the opposing arch and your bite are scanned. No tray, no putty, no gagging.

  4. 04

    Design & mill

    45 min

    Anatomy designed on screen against your own opposing tooth, then milled in the clinic from a single ceramic block.

  5. 05

    Characterise & glaze

    25 min

    Stains and translucency applied by hand, then fired. This is the step that separates a same-day crown from a plastic-looking one.

  6. 06

    Bond & adjust

    40 min

    Bonded under isolation, bite adjusted and polished. You leave with the final restoration in place.

By how much sound tooth is left after the decay and old filling come out. If the walls are intact we keep them and place an onlay. If they are undermined or cracked, covering them fully is what prevents the tooth splitting later.
For single units, yes — the milling tolerances are the same and the ceramic is the same material. Where a laboratory still wins is multi-unit anterior aesthetics, and for those cases we use one.
Shade is taken under calibrated light before preparation, when the tooth is still hydrated, and the restoration is characterised and glazed by hand. Adjacent-tooth matching is the hardest thing in dentistry, and it is where the extra twenty-five minutes goes.
Published data for bonded lithium disilicate shows above 95% survival at ten years. The commonest failure is not the ceramic but decay at the margin, which is a hygiene question rather than a materials one.
Usually not. We only treat the nerve when it is already irreversibly inflamed. Preparing a tooth for a crown does not itself require it, despite how often the two are bundled together.
